Your AMH Number Is Not Your Fertility Score. Here's What It Actually Means

AMH is not a fertility test. It was never designed to be one. It measures how many eggs you have remaining and how you are likely to respond to IVF stimulation drugs. It does not measure egg quality, and it does not predict whether you can get pregnant. A 2025 article in Fertility and Sterility, one of the most respected journals in reproductive medicine, as well as Pubmed made this explicit. And yet, every day, women receive a low AMH result and are told, directly or by implication, that their chances are over. They are not.

If you are reading this because your AMH came back lower than expected and you are trying to make sense of what it means, this article is the conversation you should have had but probably did not. We are going to be honest with you: about what AMH actually tells you, about what TCM can and cannot do, and about where realistic hope lives in all of this.

What AMH Actually Measures (and What It Does Not)

Anti-Müllerian hormone is produced by the small follicles in your ovaries. Its level in your blood correlates with the number of eggs you have remaining, your ovarian reserve. A typical AMH for a fertile woman is between 1.0 and 4.0 ng/ml. Below 1.0 is generally considered low. Below 0.5 is very low.

What AMH is genuinely useful for is predicting how you will respond to IVF stimulation drugs. A woman with lower AMH will likely produce fewer eggs during a retrieval cycle, which helps your fertility specialist choose the right protocol and set appropriate expectations for IVF outcomes. In that context, AMH is a valuable clinical tool.

What AMH does not measure is whether you can get pregnant.

A systematic review of over 4,388 women published in Frontiers in Endocrinology concluded that serum AMH levels have poor predictive value for natural pregnancy, in both younger and older subgroups. The researchers noted that compared to ovarian reserve, regular ovulation and oocyte quality may hold greater significance in spontaneous conception. Separately, live pregnancies have been documented in women whose AMH was undetectable.

This does not mean AMH is irrelevant. It means it is being used to answer a question it was never designed for. Your AMH tells your IVF specialist how to dose your protocol. It does not tell you whether motherhood is possible. Understanding this distinction can change everything about how you approach the next stage of your journey.

It is also worth knowing that AMH is not a fixed, permanent number. It can fluctuate based on stress, illness, vitamin D levels, hormonal contraceptive use, and timing within your cycle. A single low reading is not necessarily a definitive assessment of your ovarian reserve.

What Low Ovarian Reserve Actually Means for Your Fertility

A diagnosis of diminished ovarian reserve (DOR) means you have fewer eggs remaining than expected for your age. It does not mean your eggs are all poor quality. It does not mean you cannot ovulate. And it does not mean you cannot conceive.

What it does mean is that the margin is narrower. You have fewer cycles to work with, fewer eggs per cycle, and potentially fewer years before the reserve depletes further. This changes the urgency and the strategy, but it does not change the fundamental biology: you only need one good egg to conceive.

The critical distinction that often gets lost in the anxiety of a low AMH result is the difference between egg quantity and egg quality. AMH and antral follicle count measure quantity. But quality, whether your eggs carry the correct number of chromosomes, whether their mitochondria are producing sufficient energy, whether they can support fertilisation and early embryo development, is determined by different factors entirely. And those factors are ones you can influence.

What TCM Can Do (Honestly)

Let us be direct about this, because honesty is more valuable than hype.

TCM cannot create new eggs. No treatment, whether Western or traditional, can increase the total number of eggs in your ovaries. That supply was determined before you were born, and it only decreases over time.

TCM is unlikely to dramatically raise your AMH number. Some studies show modest improvements. A 2023 systematic review and meta-analysis of 13 randomised controlled trials found that acupuncture significantly improved AMH levels, increased antral follicle count, and reduced FSH in women with diminished ovarian reserve. Combining acupuncture with Chinese herbal medicine was more effective than acupuncture alone. A 2025 review published in PMC found that TCM can improve ovarian function through multiple mechanisms including antioxidation, immune regulation, and cellular signalling pathways. These are meaningful findings. But "improved" does not mean "reversed," and any clinic promising to restore your ovarian reserve to youthful levels is not being truthful with you.

What TCM can do, and where its real value lies, is optimising the quality and environment of the eggs you still have.

This is the crucial point. When your reserve is low, every individual egg carries proportionally greater weight. If you are producing two or three follicles per cycle instead of ten, then the quality of each one matters enormously. The 90-day egg maturation window, the period during which each egg develops from dormant follicle to ovulation-ready oocyte, becomes the highest-leverage intervention available to you. During this 90-day window, TCM works to:

  • Improve blood flow to the ovaries. Acupuncture stimulates specific points along the Kidney, Spleen, and Liver meridians to enhance pelvic circulation. Better blood flow means better delivery of oxygen, nutrients, and hormonal signals to the developing follicles.
  • Nourish Kidney Jing and slow its depletion. Chinese herbal medicine uses deep, nourishing formulas tailored to your specific pattern. If Yin is deficient, moistening and cooling herbs are prescribed. If Yang is deficient, warming and tonifying herbs are used. If both are involved (which is common in DOR), the formula addresses both.
  • Support the luteal phase. A shortening luteal phase is one of the earliest signs of declining reserve. Moxibustion and Yang-tonifying herbs help sustain progesterone and create conditions for implantation. If the pattern involves cold in the uterus, our article on Gong Han explores this in depth.
  • Reduce oxidative stress. Oxidative damage is one of the primary mechanisms behind age-related egg quality decline. TCM herbs with antioxidant properties, combined with dietary guidance (covered in our TCM fertility foods guide), help protect the eggs during their maturation.
  • Calm the nervous system. A low AMH diagnosis is profoundly stressful, and that stress creates a physiological cascade (elevated cortisol, suppressed reproductive hormones) that compounds the challenge. As we have explored in our article on stress and fertility, acupuncture is one of the most effective tools for shifting the body out of chronic fight-or-flight and into a state where reproduction is biologically supported.

How TCM Understands Low Ovarian Reserve

In TCM, diminished ovarian reserve maps to a pattern of Kidney Jing (essence) depletion. Jing is the fundamental substance that governs your reproductive capacity, your constitutional vitality, and your body's deepest energy reserves. It naturally declines with age, which is why ovarian reserve decreases over time. This is consistent with the TCM observation that a woman's Jing peaks at 28 and begins its decline at 35.

But the rate at which Jing depletes is not purely a function of age. In TCM, it is influenced by constitution (some women are born with stronger Jing reserves than others), lifestyle (chronic overwork, poor sleep, excessive stress, and poor diet all accelerate depletion), health history (serious illness, surgery, or chemotherapy can deplete Jing rapidly), and emotional state (prolonged grief, anxiety, or emotional exhaustion drains Jing over time).

This is why two women of the same age with the same AMH can have very different fertility outcomes. Their numbers are similar, but their internal environments are not. TCM assesses that internal environment, the state of your Jing, the quality of your blood, the flow of your Qi, the warmth of your uterus, and identifies where the body can be supported.

Most women with low ovarian reserve present with a combination of patterns. Kidney Yang deficiency (cold extremities, fatigue, weak lower back, shortened luteal phase) and Kidney Yin deficiency (night sweats, restlessness, dryness, scanty periods) are the most common foundations. Layered on top, you may see Liver Qi stagnation from the stress of the diagnosis itself, blood deficiency from insufficient nourishment, or dampness from poor digestive function. If You Are Considering or Preparing for IVF

For many women with low ovarian reserve, IVF becomes part of the conversation. This is where AMH is genuinely useful: it helps your specialist predict your response to stimulation and plan accordingly.

TCM and IVF work well together here, and the case for combining them is arguably strongest in women with DOR. If your IVF cycle is likely to retrieve only a small number of eggs, then the quality of each egg retrieved matters immensely. Starting TCM treatment at least three months before your stimulation cycle gives your body a full 90-day window to optimise the eggs that will be collected. During the IVF cycle itself, acupuncture can support follicle response during stimulation, reduce medication side effects, improve uterine receptivity before transfer, and calm the nervous system throughout. After transfer, gentle treatment supports the luteal phase and early implantation.

For women doing IVF with low reserve, this preparation is not a luxury add-on. It is arguably the most impactful thing you can do to maximise the outcome of each cycle, and when each cycle represents significant financial and emotional investment, that matters.

What About Natural Conception?

This depends on your individual situation, and it is a conversation worth having honestly with both your fertility specialist and your TCM physician.

If you are under 38, still ovulating regularly, have no other significant infertility factors, and your AMH is low but not critically low, natural conception with TCM support may be a realistic option. The research is clear that AMH has poor predictive value for spontaneous pregnancy, meaning a low number alone should not rule out trying naturally.

If you are over 40, have very low or undetectable AMH, or have additional factors (tubal issues, male factor, endometriosis), then IVF with TCM preparation may be the more practical path. This is not a failure. It is a strategic decision, and TCM supports it fully.

The key is making the decision from a place of information and calm, not from the panic that a single lab number often triggers.

How Long Does Treatment Take?

Consistent with the 90-day egg maturation cycle, our physicians typically recommend a minimum of three months of treatment. For women with deeper Jing depletion, a longer course may be beneficial.

Symptomatic improvements often appear earlier: better sleep, warmer extremities, reduced PMS, improved menstrual blood quality, and greater emotional resilience. These are not just comfort markers. They are clinical signs that the internal environment is shifting.

For women preparing for IVF, measurable outcomes include improved follicle response during stimulation, better egg quality at retrieval, and stronger embryo development. For those trying naturally, the clearest indicators are improved BBT charts (stronger luteal phase, clearer ovulation patterns), healthier menstrual cycles, and ultimately, conception.
