If your IVF cycle costs thousands of dollars and months of emotional energy, why would you walk into it without preparing the one thing the clinic cannot control: the condition of your body? Your fertility specialist handles the medical protocol. The stimulation drugs, the monitoring scans, the retrieval, the transfer. That part is well covered. But the quality of your eggs, the thickness of your uterine lining, the blood flow to your reproductive organs, your stress levels, your sleep, your digestion: these are shaped by what happens in the months before stimulation begins. And this is exactly where most women have no plan at all. In Singapore, the government co-funds up to 75% of assisted reproduction costs for eligible women, but each cycle still represents a significant financial and emotional investment. Maximising every cycle matters. A retrospective cohort study published in Reproductive BioMedicine Online found that women who received whole-systems TCM alongside IVF (acupuncture, herbal medicine, and dietary and lifestyle guidance over multiple months) had more than double the odds of a live birth compared to IVF alone. Not from one session on transfer day. From months of preparation. Here is what that preparation actually looks like, broken down by stage.

6 Months Before: Laying the Foundation

If you have the luxury of time, this is the ideal starting point. Six months out, the focus is entirely on building your foundational health and addressing any underlying imbalances that could affect your IVF outcome.

At this stage, our TCM physicians will conduct a thorough assessment of your constitution, your menstrual cycle, your pulse, your tongue, and your health history. The goal is to identify your TCM pattern: are you dealing with Kidney Yang deficiency (cold constitution, fatigue, cold extremities)? Kidney Yin deficiency (night sweats, dryness, heat sensations)? Liver Qi stagnation from chronic stress? Blood deficiency? Dampness clogging the system? Most women have a combination, and this assessment determines everything that follows.

Treatment at this stage typically includes:

  • Weekly or fortnightly acupuncture to begin regulating your cycle, improving pelvic blood flow, and calming the nervous system
  • Chinese herbal medicine tailored to your pattern, focusing on nourishing Kidney Jing (essence), building blood, and resolving any stagnation or dampness
  • Dietary and lifestyle guidance to address the habits that may be quietly undermining your fertility (we have written in more detail about this in our guide to fertility-boosting foods in TCM)

This is also the time to address anything else that could affect your cycle. If you struggle with insomnia, chronic stress, digestive issues, or period pain, these are not separate problems. In TCM, they are all connected to the same organ systems (Kidney, Liver, Spleen) that govern your reproductive health. Treating them now improves your overall readiness.

If you have had previous failed IVF cycles, this extended preparation window is especially valuable. It gives your body time to recover from the hormonal load of previous stimulation, rebuild depleted reserves, and reset before going again.

3 Months Before: The Critical Egg Quality Window

This is the most important phase, and the minimum preparation window we recommend for every patient.

Why three months? Because each egg takes approximately 90 days to mature from a dormant follicle to an ovulation-ready oocyte. The eggs that will be retrieved during your IVF cycle are developing right now, influenced by your blood flow, your nutrition, your hormonal environment, and your overall health. What you do during this 90-day window directly affects the quality of the eggs your fertility specialist will retrieve.

At this stage, treatment intensifies:

  • Acupuncture increases to weekly sessions, focused on improving ovarian blood flow, supporting follicle development, and optimising hormonal balance
  • Herbal formulas shift to focus specifically on nourishing Kidney Jing and building blood, the two TCM foundations of egg quality
  • If moxibustion is appropriate for your pattern (particularly if Yang deficiency or cold is present), it is incorporated to warm the uterus and improve circulation to the reproductive organs
  • Supplements may be discussed alongside your herbal prescription. CoQ10 (ubiquinol), vitamin D, and folate are among the most researched for egg quality, and your TCM physician can advise on how these fit alongside your herbal formula without conflict

This is also when we start paying close attention to your menstrual cycle as a diagnostic tool. The colour, volume, texture, and timing of your period tell us a great deal about how your body is responding. Dark, clotty blood with pain may indicate blood stasis. Very light, pale flow may point to blood deficiency. Short cycles or a short luteal phase may suggest insufficient Yang energy. These observations guide ongoing adjustments to your treatment plan.

If you have been diagnosed with a specific condition such as PCOS, endometriosis, or diminished ovarian reserve, this three-month window is where targeted TCM treatment can make a measurable difference in how your body responds to stimulation.

6 Weeks Before: Fine-Tuning

By now, the foundational work is done. Your cycle should be more regular, your energy levels improved, and your body in a significantly better position than when you started.

The six-week mark is about fine-tuning. Treatment continues weekly, but the focus narrows:

  • Acupuncture points are selected to specifically support uterine lining development and ensure strong blood flow to the pelvis
  • Herbal formulas may be adjusted to prepare for the transition into your IVF cycle. Some herbs will be modified or paused once stimulation drugs begin (your TCM physician will discuss this with you and, where possible, coordinate with your fertility clinic)
  • Stress management becomes a priority. The weeks leading into an IVF cycle are emotionally loaded, and elevated cortisol disrupts the hormonal signals your body needs for a good response to stimulation. Acupuncture is particularly effective here, helping to regulate the nervous system and keep your body in a calmer, more receptive state
  • Sleep quality is reviewed and optimised. In TCM, sleep is when the body replenishes Yin and blood. If your sleep has been disrupted, now is the time to address it

During Stimulation: Supporting Your Body's Response

Once your stimulation drugs begin, the role of TCM shifts. You are now under the active care of your fertility clinic, and TCM works alongside the medical protocol rather than leading it.

Acupuncture during stimulation focuses on:

  • Reducing the side effects of hormonal medication (bloating, headaches, mood swings, insomnia)
  • Supporting follicle growth and response to medication
  • Managing anxiety, which tends to spike during this phase as you move from scan to scan
  • Maintaining good blood flow to the ovaries and uterus
  • Sessions are typically scheduled once or twice during the stimulation phase, carefully timed around your monitoring appointments.

Regarding herbal medicine during stimulation: this is a clinical decision that varies by patient and by protocol. Some physicians pause herbal formulas entirely during stimulation, while others adjust to a gentler prescription that supports the body without interfering with the IVF drugs. Transfer Week: Optimising Receptivity

Embryo transfer is the moment everything has been building towards, and TCM has a specific role to play here.

A 2026 meta-analysis published in Frontiers in Endocrinology found that acupuncture increased clinical pregnancy rates by 25% and embryo implantation rates by 30% in women with diminished ovarian reserve undergoing IVF. The most commonly studied protocol involves acupuncture sessions immediately before and after embryo transfer, targeting points that promote uterine receptivity, improve blood flow to the endometrium, and calm the nervous system.

After Transfer: The Two-Week Wait

The two weeks between transfer and your pregnancy test are some of the most emotionally challenging days of the entire process. TCM can help here too.

Acupuncture during the luteal phase focuses on supporting implantation and early pregnancy by nourishing Kidney Yang (which sustains progesterone levels), calming the mind, and maintaining good circulation to the uterus. One to two sessions during this window can make a meaningful difference to both your physical state and your emotional wellbeing.

If your physician has paused herbal medicine during stimulation, a gentle formula to support the luteal phase may be reintroduced at this stage, depending on your situation.

Beyond treatment, this is also a time to be kind to yourself. Rest when you can. Eat warm, nourishing meals. Avoid intense exercise. Limit your exposure to stressful situations where possible. Your body is doing something extraordinary, and it deserves your care.

What If You Do Not Have 6 Months?

Not everyone has the luxury of a long preparation window, and that is okay. Even if your IVF cycle is weeks away, there is still meaningful work we can do. And the stress reduction, sleep support, and dietary guidance that TCM provides are valuable at any point.
